Requirements for walk-in exam:
1. Travel Authority
2. Health Certificate
3. ID Card
4. Test permit/filled-out application form with 2 pcs 1×1 ID picture
5. Photocopy of your report card
6. Black or blue pen
7. Alcohol for personal use

All TAKERS MUST wear a FACE MASK.
Online registration via doorstep.cmu.edu.ph is now available.
