REQUEST OF RECORDS

Secure a request form at the Office of the University Registrar (OUR)-Window 1.

Fill out the request form properly and completely. Use the name indicated on your PSA-issued Birth Certificate.
*For married female, fill-in both your maiden and married name on the space provided.
Submit the accomplished form at the OUR-Window 1 or attach the said form and email it to onlinerequest@cmu.edu.ph
 
If the accomplished form is sent through email, kindly monitor your email as the in-charge will reach-out and give feedback through your email.

TOR is Php 150.00 for the first 2 pages and Php 80.00/page on the succeeding pages.
Certification is Php 80.00.
Authentication is Php 80.00 per set (2 copies per set)
Diploma is Php 150.00.
Certificate of Transfer Credentials (HD) with TOR for Evaluation is Php 560.00 (for 3 pages TOR);
Php 400 (for 2 pages TOR).

Once the requirements and payment is complete, you will be given a CLAIM SLIP of when is your due date to claim the requested records. (1-3 days only except for peak season).
Please note of the following:
Your request will be processed upon submission of the Official Receipt and required credentials.
Please be informed that we don’t have mailing services yet. You need to personally claim the requested records at the Office of the University Registrar Window 1 or your authorized representative.
 
When a third party is the one who claim the requested records, the authorized representative must submit the following:
 
  • Claim Slip
  • Authorization letter
  • Notarized authorization letter for CAV requests (local employment)
  • Special Power of Attorney (SPA) for CAV requests (overseas employment)
  • Photocopy of the valid ID of the owner
  • Valid ID of the authorized representative
How to apply for Certification, Authentication and Verification (CAV)?

 

Secure a request form at the Office of the University Registrar (OUR)-Window 1 (You may also download the form here).
Fill out the request form properly and completely (please indicate if CAV is for Overseas or Local Employment)
Submit the accomplished form at the OUR-Window 1 for processing and assessment.
 
The officer-in-charge may ask you of the following requirements:
 

For Local Employment

  • Original Transcript of Records with remarks “Issued for Local Employment Purposes”
  • Original Diploma (for graduate students)

For Overseas Employment

  • Original Transcript of Records with remarks “Issued for Overseas Employment Purposes)
  • Original Diploma (for graduate students)
In cases wherein you need to request for your Transcript of Records and Diploma, kindly refer to the process on how to get a Transcript of Records and other documents.

 

Payment Slip will be given by the OUR in-charge. Pay the fee at the Cashier’s Office or you may opt to pay online.

CAV is Php 80.00 (in a separate Official Receipt)
Authentication is Php 80.00/set (2 copies per set)

 

Note: For multiple requests (TOR, CAV, Certification), payment for CAV must be in a separate OR as the OR Number will be reflected on the CAV.

 

Submit the required documents and the Official Receipt at the OUR-Window 1.
Once the requirements and payment is complete, you may claim your CAV within the day (if the original TOR and Diploma is hand carried by the client at the time the request has been made).
In case the client requested for TOR, Diploma and CAV at the same time, due date/claim date of the requested records/documents is 5-7 days (depending on the availability of the signatory of the Diploma).

  1. How to claim the requested TOR and Certifications?

Upon evaluation of your records, you may be required to submit any of the following:

Php 150.00 for the first 2 pages, and Php 80.00/page for the succeeding pages. If your TOR is 3 pages, you need for pay Php 230.00.

5. How to request for an INC Form?

You may request for an INC Form at the Office of the University Registrar Window 3.
 
6. How much is the INC Form?

Php 30.00 per subject

7. How to request the Withdrawal Form?

You may request for the Withdrawal Form at the Office of the University Registrar Window 2.

8. How much is the Withdrawal Form?

Php 50.00 per transaction

9. How to request for Grade Slip and Certificate of Registration (COR)?

You may request for your Grade Slip and Certificate of Registration (COR) at the Office of the University Registrar Window 2, Window 3, Window 5, and Window 6

Required Laboratory Tests:
5.4.2 Tuberculosis Test (chest x-ray) – should be taken within or during
the enrollment period
5.4.3 Hepatitis A Ag Test – taken within 6 months
5.4.4 Drug Test – taken within 6 months from any accredited drug testing
center
5.4.5 Pregnancy Test (for female applicants) – should be taken within or
during the enrollment period

A bonafide foreign student must comply with the school’s requirements, including the
submission of the following documents:

  1. Application letter addressed to the University Registrar, Central Mindanao University;
  2. Documents listed below duly authenticated by the Philippine Foreign Service Post (PFSP) of the student-applicant’s country of residence. The authenticated copies must bear the diplomatic red ribbon, original signature and seal of the PFSP abroad; 

    2.1. Transcript of Records from high school and college attended (must be translated in
    English)
    2.2. Diploma (must be translated in English)
    2.3. Birth Certificate (must be translated in English)
    2.4. Two (2) Certificate of Good Moral Character or letter of reference/recommendation
    from the school last attended and from a prominent person of the community (must
    be translated in English)
    2.5. Police Clearance Certificate issued by the National Police Authorities in the
    student’s country of origin or residence (must be translated in English)
    2.6. Certificate of Eligibility to pursue bachelor’s degree – should clearly indicate that the
    student is a graduate of Senior High School and are eligible to pursue a bachelor’s
    degree in the Philippines. The “Certificate of Eligibility to Pursue Bachelor’s Degree”
    may be secured from any of the following: (a) the school where the applicant
    graduated his/her Senior High School, (b) your country’s Ministry of Education or its
    equivalent, (c) Philippine Embassy in the applicant’s country of origin (must be
    translated in English)
    2.7. Certification of English Language Proficiency Test i.e. IELTS, TOFEL or any
    equivalent certification (if available is an advantage but not required)

  3. Notarized affidavit of support (with bank statement) or Letter of Guarantee from sponsoring company/organization from abroad (must be translated in English)
  4. Personal History Statement (PHS), duly accomplished and signed by the applicant in English and in national alphabet, accompanied by personal seal, if any, original left and right hand prints on PHS forms and original 2”x2” or passport-size pictures
  5. International Student Fee of US$300 per semester and US$150 for Midyear in its Philippine Peso Equivalent (to be paid at the Cashier’s Office upon enrollment)
