HOW TO GET AN OFFICIAL TRANSCRIPT OF RECORDS (TOR) AND OTHER DOCUMENTS?
- Secure an application form at the Office of the University Registrar (window 1).
- Fill out the application form properly and completely. Use the name based on your birth certificate from NSO; For married female student, indicate the family name you used during your last enrollment in CMU.
If shiftee, indicate the college(s) last attended. - Submit your accomplished form at the OUR counter (window 1) for processing and assessment.
- Pay fee at the Cashier’s Office. (TOR fee is PhP 150.00/first 2 pages and Php 80.00/succeeding page
- Submit at the OUR Counter :
- Properly accomplished form
- Official Receipt (OR) of payment
- Requirements required by the Officer in charge in the Records Room of the University Registrar’s Office such as:
For Undergraduate Students
- All admission requirements must be completed
Admission Requirements:
- Notice of Result (CMUCAT)
- Original Form 138 (High School Report Card) and one (1) photocopy
- Photocopy of Parents’ Income Tax Return (for scholarship only)
- PSA Original Copy of Birth Certificate (LCR form 1A is required if PSA copy is not clear) and one (1) photocopy
- Four (4) copies 2×2 colored pictures (with collar and white background)
- Certificate of Good Moral Character
- Freshmen Orientation Certificate
- Marriage Certificate (for married women)
- Certificate of Transfer Credentials or Honorable Dismissal (for transferee)
- Official Transcript of Records (for transferee)
- Police Clearance (for transferee)
- General Clearance
- 2×2 picture (w/ collar & white background)
For Graduate Students
- All admission requirements must be completed
Admission Requirements:
- Notice of Result (GSAT)
- Notice of Admission
- Certificate of Transfer Credentials or Honorable Dismissal
- Transcript of Records for Evaluation
- PSA Original Copy of Birth Certificate (LCR form 1-A is required if PSA copy is not clear) and one (1) photocopy
- Marriage Certificate (for married women)
- Major Department Evaluation
- 2×2 picture (w/ collar & white background)
- Plan of Course Work (to be submitted upon Application for Graduation)
How to apply for Certification, Authentication and Verification (CAV)?
- CAV is attached to the original or photocopy of documents for authentication by the Department of Foreign Affairs (DFA).
Requirements:
- Original Transcript of Records with clear purpose indicated at the bottom of the last page:
“Issued for Local Employment Purposes” if applying locally.
“Issued for Overseas employment Purposes” if applying abroad. - Original Diploma
- Official Receipt from the Cashier (PhP 160.00):
Official Receipt should be separated:
PhP 80.00 for authentication
PhP 80.00 for the CAV
How do I claim the documents that I applied for?
- Identification card.
- When claiming for a third party, submit a letter of authorization together with an ID card from the owner of the document(s) to be claimed and the ID of the representative.
Points to remember
- TOR’s and other documents not claimed within three (3) months are subject for disposal.
