𝗣𝗟𝗘𝗔𝗦𝗘 𝗥𝗘𝗔𝗗 𝗔𝗡𝗗 𝗙𝗢𝗟𝗟𝗢𝗪 𝗧𝗛𝗘 𝗜𝗡𝗦𝗧𝗥𝗨𝗖𝗧𝗜𝗢𝗡𝗦 𝗔𝗡𝗗 𝗦𝗧𝗘𝗣𝗦 𝗧𝗛𝗢𝗥𝗢𝗨𝗚𝗛𝗟𝗬

2. Click “Register” button
3. Select ULHSAT to confirm your identity as one of the takers
4. Input the application ID number found in your test permit
5. Click “Register” button. If your application ID number and name for registration did not match, do not register. Please email your name to oasp@cmu.edu.ph, and wait for further instruction

Print Form 1: Notice of Results. You need to fill-out the student profile in order to print Form 2: Confirmation Slip.
