2. Must pass trainability/aptitude test
3. Physically, emotionally and mentally fit
4. Can communicate effectively both orally and written from
5. Can perform basic mathematical computation
Additional Admission Requirements for TESDA program (Health Care Services NCII):
1. Photocopy of Form 138
2. High School Graduate
3. NSO Birth Certificate
4. 4 pcs. 2×2 picture (with collar & white background)
5. 1 long brown envelope
6. Accomplished Student Health Assessment Record
