[1] Applicants with printed application forms, please visit the Office of Admissions, Scholarships, and Placement (OASP), Monday-Saturday 8:00 am-5:00 pm for approval

Application Form must have the following:
a. Complete signatories (Principal/Guidance Councilor and Applicant)
b. 2 pcs. of 1×1 I.D. picture glued in appropriate boxes.

[2] Applicants who cannot access devops.cmu.edu.ph/doorstep, download the form https://rb.gy/heexo3, print, fill out all information needed, and proceed with #1.
